I got it because I read some story that Madagascar had isolated iteself during one of the SARS/Bird/Swine Flu things and assumed that is what it meant. Have no clue about the whole video game aspect.

It was some game where you had to design a virus and then let it loose on the world. Only Madagascar would close its ports everytime shit went down so every game you'd normally win would end with the entire world infected except Madagascar, thus no win. At least that's how I remember it.
There were some awesome jokes about it, but yeah, you had to have been there.
