I don't, but I think I'm going to start. Any suggestions?

I just use cosmos. It can be buggy at times but overall it's a great improvement. So much so that Blizzard is stealing some of their ideas.
Which leads me to think that might have been the point all along.

I ditched cosmos and installed a bit of gypsy mod instead and a couple of stand alone mods. I like that I can make everything tiny with gypsy mod.

With the new tool to select which parts of a mod you want, I use the good stuff from Cosmos plus the Gypsy character/group displays. Total, I think I have maybe 15 modules selected out of the list from Cosmos and Gyspy, 3 of them from Gypsy.
Btw, they help a lot! Playing a rogue is so much better because it'll actually switch toolbars when you enter Stealth mode, giving you a place to place all of your stealth-only actions. This also works for warriors and their stances.
All kinds of goodies - and if you find you don't use something, just uncheck it from the list at character selection.

The UI mods are amazing, and some really helpful. I liked the "carebear" mod which automatically refuses duel requests and will automatically trigger an emote of my choosing when doing so.

I use Cosmos, dislike playing without it. Favorite mod: Tooltip info on the mouse. Also dig extra toolbars, I don't get too fancy, I have most other submods shut off.

I like having the option of uploading my profile to Thottbot, like Planetside and their linkable web stats for your character. But I don't like the idea of creating an account with them, especially if they are owned by IGE as the rumor has it.

You can do the same thing with WoWReader from Allakhazam- all your characters will be attached to your login so you can view them offline.

There are loads of great mods, but if there's one that I'd recommend to anyone, it's Telo's LootLink. Shift-clickable info on every item that you encounter in game, be it at the auction house, inspecting another player, loot, etc, will be stored so that you can view it or share it any time you choose. I have over 8500 items in mine now, I think it's pretty great.
I do think some of the sorting features could stand some improvement but maybe they'll get to it in time.
My apologies for...well, I just lurk here really, I didn't necessarily mean to pop up with a sales pitch or whatever the first (edit: erm, 2nd) time I posted. I just think it's a very useful add-on and I'm sure that anyone who has tried it probably agrees.

You'd be a sad sad little man if you did go around to forums pimping a free addon for a game, so it's pretty unlikely anyone would accuse you of such. :P Anyway, I use n old verion of the nurfed UI ( www.nurfed.com) which is mostly a compilation of other mods with a little extra customization. The main things I like from it is the bottombar, sidebar, gatherer, clock, buff timers, quest speedup, quest levels, and ading numbers and percentages to my health/mana. I will soon be dropping it for my own compilation soon though.

Well the extent of creating an account is just providing a username and a password.

Besides Cosmos, I use RaidBar and CQMC.
RaidBar is the kind of UI mod that's going to hugely affect the nature of PVP in WoW. In case you don't know what it is, it's a mod that allows you to create a targeting bar for anyone else using RaidBar, allowing you to monitor, quickly target, and cast beneficial spells on players out of group who're also using RaidBar. It's already made MC/Onyxia raids so much easier.
Now in PvP, instead of the focus being '5v5' or being part of a '5-man gank squad', huge raid-style battles will be more popular(not withstanding lag issues) because it'll be possible for separately-grouped healers to support one another and players out of group. Due to the nature of the dispel spells for shamans and paladins, Horde priests were more vulnerable to polymorph than Alliance ones, and that won't be the case anymore with RaidBar if a raid set of priests watch out for one another. Instead of having 1 or 2 healers watching out for you(in the case of a standard 5 man), you can potentially have 10 or more healers keeping an eye out for you.
