Oh. Well, technically it's 6 lines. But what the hell, I'm at work. Normal = Normal, levels 1-30 Elite = Harder Normal, levels 1-30, unlocked after Normal, only new characters can be on this Nightmare = Hell difficulty. Levels 30-50. Unlocked for individual characters after they finish the game. Nightmare Elite = Unlocked on Elite characters after they finish. This is known as "die" mode for most characters. They seem to be a much more common drop than the flaming skull was.
