I've seen awful ones. I've seen people bounce it up there a lot. I've never seen someone cricket style it like they were trying to hit the Bull.

The only time I've seen a pitcher do that is when time got called late.

Head Like A Hole is one of my all time favourite songs. That mashup should feel like sacreledge. But it just works. Doesn't diminish my love for the original; seeing NIN play it live on their last tour remains one of the best live experiences I've ever had, it was like a physical assault. But the silliness of that mashup gets to me. I love it when two extremes meet.

Maybe. Her form isn't bad (at least from the back). The "woman throw" tends to be more of a push which she didn't do.

However what she did do was hold the ball in her hand rather than in her fingers. If you hold the ball in your fingers as your arm comes through the ball will roll off your fingers unless you make a really determined effort to stop that. If you hold the ball in your hand your palm will snap down and take the ball with it, unless you make a really determined effort to stop that (this is how you throw a change-up)
